Acute Severe Mitral Regurgitation

Abrupt regurgitation into a relatively noncompliant left atrium produces high wedge pressure, a large v wave and reduced forward output.

Overview

Select Acute Severe Mitral Regurgitation under Clinical controls → Valves. The bold italic live label confirms the active selection. These are the preset’s teaching features; combined disease, rate and interventions can change the resulting numbers.

Abrupt regurgitation into a relatively noncompliant left atrium produces high wedge pressure, a large v wave and reduced forward output.

Acute MR can cause pulmonary edema without chronic chamber enlargement.

An eccentric jet directed toward a right pulmonary vein can cause unilateral, often right upper lobe edema. The CXR preset illustrates right-predominant bilateral edema.

The wedge mean can substantially exceed LV end-diastolic pressure; a v wave is not a measure of regurgitant severity.

Step by step

  1. Reset interventions if you want the untreated example.
  2. Select the condition and inspect the pressure summary and relevant waveforms.
  3. Pause for annotations; add one intervention or loading change to compare the response.
